Treatment resistant depression is more than persistent low mood. It is depression that has continued despite two or more adequate trials of oral antidepressant therapy, and it touches sleep, energy, concentration, relationships, and work. Spravato (esketamine) is an FDA approved nasal spray used in combination with an oral antidepressant for adults with this condition. Because of the side effect profile, it is administered only in REMS-certified clinics under direct medical supervision. Our clinic is certified to evaluate, induct, and continue Spravato therapy for adults in Las Vegas.

Who this service is for

Adults, 18 and over, with major depressive disorder who have not adequately responded to two or more antidepressants at therapeutic dose and duration. We also evaluate referrals from outside psychiatrists who want to add Spravato to an existing medication plan.

What the evaluation looks like

Before any Spravato session, you complete a full psychiatric evaluation with our team. We review your depression history, prior medication trials, medical history, and any contraindications. We confirm that an oral antidepressant is on board and at an appropriate dose. We discuss what to expect during a session, including dissociation, transient blood pressure elevation, and the post dose monitoring period. If you and your clinician decide Spravato is the right next step, we begin the insurance authorization and scheduling.

What to expect at a Spravato session

• You arrive after a light meal and check in with our clinical team
• You self administer the nasal spray under direct supervision
• We monitor blood pressure, sedation, and dissociation for at least two hours
• You are discharged to a licensed driver, not to drive yourself
• The induction schedule is twice weekly for the first month, then weekly, then less frequent maintenance dosing based on response

In-house diagnostics

We draw any baseline labs needed before starting therapy and have a 12 lead EKG on site. Blood pressure is monitored throughout each session. Because our psychiatry, internal medicine, and pulmonology teams share a record, any medical question that comes up during therapy can be handled without sending you out of the building.

Working with your other clinicians

If you are already established with a therapist or outside psychiatrist, we coordinate directly. Many of our Spravato patients also work with our psychiatry and psychology team for combined medication management and therapy.

Insurance and access

Many commercial and Medicare plans cover Spravato when criteria are met. We verify benefits before your first dose so there are no surprises. We accept most major insurance plans. Humana and HPN are not accepted at this time. Se habla español.
