Most people with high blood pressure can be successfully managed by their primary care physician with lifestyle changes and one or two well-chosen medications. The patients we typically see in nephrology are the ones for whom that approach hasn't been enough, three or more medications and the numbers still aren't where they should be, or blood pressure that swings unpredictably, or readings that point to a specific underlying cause worth investigating.
When to consider a specialist evaluation
Consider a nephrology evaluation if you are on three or more antihypertensive medications and still above goal, if your blood pressure rose suddenly or unusually young, if you have unexplained low potassium, or if imaging or symptoms suggest a secondary cause such as renovascular disease or primary aldosteronism.
What we evaluate
We review your full medication list (including over-the-counter drugs and supplements), check for white-coat effect with home or ambulatory monitoring when indicated, evaluate kidney function and electrolytes, screen for secondary causes when the picture warrants it, and address the contribution of obstructive sleep apnea, which is one of the most commonly missed drivers of resistant hypertension. We coordinate internally with our pulmonology team when sleep apnea evaluation is the next step.
A plan you can actually follow
Treatment regimens that look elegant on paper but require a fistful of pills twice a day are the regimens patients abandon. We work to build a simpler, lower-burden plan: combination pills where appropriate, doses timed to your routine, and clear instructions on what to do when home readings change.
